Website information and insurance-related records may involve different service providers and notices. Ask Todd which privacy information applies to your quote, application, or existing policy.
Information you choose to provide
When you use an intake or service form, the form provider receives the information you enter or submit through that service. The current Customer Intake requests contact details, address, date of birth, household information, education and occupation, information about a co-insured person, and contact preferences. Other requested fields depend on the form and your insurance needs. Review the fields and any notices shown in the form before providing information. If you call Todd, you can choose what information to discuss.
Contact and insurance information you provide is intended to support your inquiry, quote request, application, or service request. Ask Todd before sending sensitive records through a channel that has not been designated for them.
Forms and service providers
- RiskAdvisor: The Start Intake button opens a form hosted at quoteforms.app. Opening the form connects your browser to the form service. Information entered into that form is handled through that service. Review any privacy or consent information shown in the form, or ask Todd for the applicable provider notice before submitting.
- Cognito Forms: Some older quote and service URLs still display Cognito-hosted forms. If you use one of those pages, the form is handled by Cognito rather than the Start Intake form. See the Cognito Forms Privacy Policy.
- Cloudflare: The website is hosted and delivered through Cloudflare. Loading a page sends the technical request information needed to deliver that page, including your IP address and browser request details. See Cloudflare's Privacy Policy.
- Google Fonts: The website loads fonts from Google's font services, which receive the browser requests needed to supply those fonts. See Google's Privacy Policy.
Provider notices describe those providers' practices; they do not replace information about how the agency uses insurance records. Todd can clarify the agency and insurer recipients relevant to your request.
Website analytics and cookies
This website uses Matomo analytics, with requests sent to matomo.localnerds.co, to measure website use. The current configuration uses analytics cookies and loads analytics when a page opens. The Google Analytics choice described below does not control Matomo.
Analytics requests can include the page URL, referring page, time of the visit, certain link clicks, browser or device information, and a visitor identifier. Your IP address is part of the network connection. Contact Todd with questions about how analytics information is handled.
For general information about this technology, see Matomo's explanation of analytics cookies. Your browser settings may let you restrict or remove cookies. Cookie controls do not control the information you choose to provide in an insurance form. The current Matomo configuration does not use browser Do Not Track signals to stop tracking.
Google Analytics choices
With your permission, this website also uses Google Analytics to measure page views, telephone-link clicks and openings of the intake panel. Google Analytics loads only after you choose Allow. You can choose Decline or change your choice using Google Analytics choices in the footer. The choice is saved in this browser for six months and applies to Google Analytics only; Matomo is described separately above.
Google Analytics can use cookies and receive browser and device information. The site's custom tracking uses the canonical page address without query strings or fragments and does not read insurance form fields. A telephone click is not proof of a completed call, and opening the intake panel is not proof of a submitted quote request. Advertising storage and personalization are disabled in this integration. Records, sharing, and privacy requests
Ask Todd which organizations receive information for your request and what record-retention requirements apply to your records.
To ask about access, correction, deletion, provider handling, or other privacy choices, call (612) 500-5215. The available response may depend on the type of record, the organization holding it, and applicable insurance or recordkeeping requirements.

Links to city sources, insurers, and other organizations lead to services governed by their own notices. This page may be updated as the agency's practices or website tools change. The date above identifies its latest revision.
